HIPEC-based cytoreductive surgery is an aggressive locoregional treatment that begins with the resection of disseminated intraabdominal disease, either completely or with minimal residual disease. After cytoreduction, chemotherapy is infused into the abdominal cavity.
Visible cancerous tumours are surgically removed from the abdominal cavity prior to cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (HIPEC). The cavity is then filled with hot chemotherapy that has been heated to 42 degrees Celsius in order to kill any remaining microscopic cancer cells.
Chemotherapy’s cytotoxicity is increased by hyperthermia, and intraperitoneal administration allows for much higher doses to be delivered locally than are possible systemically while minimizing toxicity.
